- Bring your doctor’s referral slip if you have one.
- Allow 60-65 minutes for your initial visit.
- Arrive a few minutes prior to your appointment to fill out simple paperwork, or save time and download our forms, print them, fill them out, and bring them with you.
- Wear comfortable clothes that will facilitate a physical examination and physical activities/exercises.

Q. Will my insurance cover me?
A. Insurance coverage is unique for each patient. Please contact your insurance carrier to determine what coverage you have for physical therapy.

Rules pertaining to the need for a prescription by a licensed healthcare provider are dictated by the State of Oregon, and may be complicated by your insurer. Some plans do not require a physician’s referral for the first 30 days of physical therapy.

Oregon’s Physical Therapy Licensing Board allows 60 days of treatment without a referral/prescription if insurance is not billed. If insurance is billed, the law allows us to treat for only 30 days without referral.

Q. Am I required to see a physician prior to coming to see a PT?
A. No. Your insurance company may required a MD referral (Medicare and Medicaid do), so you should check with them. Most companies allow you to come to therapy without first seeing your doctor.
